If the study team is not sure if they are doing human subject research they should refer to the UVA Non-Human Subject Research Online Tool from the IRB-HSR.
If it is obvious from the form that the study team is not doing human subject research completion of the form is optional. If it is completed, it should be kept with the study records to provide documentation that the study team investigated if the project involved human subject research.
